Responsibilities

• Provide one-on-one care to clients in their homes, including assistance with daily living activities, wound care, and medication management
• Support clients with mobility issues, such as transferring, ambulating, and using assistive devices
• Assist with meal preparation, light housekeeping, and laundry
• Maintain a safe and clean environment for clients, adhering to infection control procedures and hazard prevention strategies
• Communicate effectively with clients, families, and healthcare professionals to ensure seamless care transitions
• Monitor and report changes in client condition, and implement emergency procedures as necessary
