Job Description

In partnership with the health services team, Memory Care Caregiver is responsible for assisting the residents in Memory Care in all areas of care, engagement, and dining as assigned by the Enliven Director, or designee.

Essential Duties and Responsibilities

Adheres to and conveys the SRG philosophy of person-centered care and Genuine Hospitality. Maintains a professional appearance, manner, and approach while on duty. Wears name tag, uniform and adheres to SRG cell phone policy. Reports to work on time and as scheduled per community policies and procedures Receives report from the previous shift, reads Care Staff Assignment and End of Shift report for information and changes before providing resident care. Completes incident reports when needed. Sign out and carry equipment such as radio, tablet, and keys at onset of shift. Documents care given via CareStream; adhering to policies and procedures. Provides report to oncoming shift and documents on the End of Shift Report and CareStream before going off shift. Answers resident call lights, door, and resident alarms promptly Provides resident assistance with activities of daily living, encouraging, and promoting residents’ self-care to the extent possible as needed or assigned Assist residents with meals, housekeeping, and laundry services as needed, requested or assigned.
