Our team is dedicated to offering comfort, dignity, and support to both patients and their families. We are seeking a Hospice Home Health Aide (HHA)/Certified Nursing Assistant (CNA) to join our growing care team.

Position Summary

The Hospice HHA/CNA provides personal care services to patients in their homes or facilities under the supervision of a Registered Nurse. This role is vital in ensuring comfort, dignity, and quality of life for patients and offering support to families during their hospice journey.

Key Responsibilities

• Provide personal care, including bathing, grooming, dressing, toileting, and oral hygiene.
• Assist with mobility, transfers, and positioning for comfort and safety.
• Monitor and report changes in the patient’s condition to the supervising nurse.
• Offer companionship and emotional support to patients and families.
• Perform light housekeeping and meal preparation related to patient care needs.
• Document care provided in compliance with hospice policies and regulations.
• Maintain patient confidentiality and uphold hospice philosophy of care.

Qualifications

• Current certification as a CNA (Certified Nursing Assistant) or HHA (Home Health Aide).
• High school diploma or equivalent.
• Compassionate, reliable, and able to work independently.
• Valid driver’s license
