Job Description
### CNA – Contract to Perm (Long-Term Care)
Location
NY, 13165
Contract Dates
May 5, 2026 – August 1, 2026 (12 weeks)
Pay
$736 – $840/week
### Schedule:
- 37.5 hours/week
- 5 shifts of 7.5 hours (5x7.5s)
- Day & night shifts as scheduled
- Every other weekend required (EOW)
- Holidays required
### Position Summary & Responsibilities:
- Provide direct and indirect resident care under RN/LPN supervision
- Assist with ADLs (hygiene, dressing, toileting, mobility, feeding)
- Maintain safe, clean, and supportive resident environment
- Support mobility, transfers, positioning, and restorative care
- Monitor resident condition and report changes promptly
- Ensure documentation is completed per facility protocol
### Requirements:
- Active NYS Nursing Assistant certification required
- BLS (AHA or RQI only) required
- Minimum 1 year CNA/LTC experience required
- Must demonstrate age-specific (adult/geriatric) care competency
### Additional Details:
- Contract-to-perm opportunity
- Exposure to bloodborne pathogens possible (clinical setting)
- Works under direction of licensed nursing staff
- Must follow infection control and safety protocols
