POSITION SUMMARY

The Home Health Aide is responsible for (1) providing various patient related services to meet the personal care needs and to promote comfort and independence to acutely ill and/or chronically ill patients in their homes; and (2) recording clinical and progress notes on Anchor's EHR system. He/she works under the direct supervision of the designated supervisor and receives clinical direction from the designated Registered Nurse.

POSITION QUALIFICATIONS

• Completion of state-approved Home Health Aide training program and/or successful passing of a written and field competency evaluation program which meets state and federal guidelines.
• Ability to relate to and care for acutely ill/chronically ill patients/families and their particular problems.
• Must be at least eighteen (18) years of age.
• Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff, and interact professionally as a member of a multi-disciplinary team.
• Must possess a strong commitment to the goals, mission, and philosophy of the organization.
