CNA/ In -Home PRN

Join our compassionate healthcare team as a CNA (Certified Nursing Assistant) / In-Home PRN (Pro Re Nata, or as needed)! In this vital role, you will provide personalized, high-quality care to clients in their homes, supporting their health, safety, and comfort. Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
• Monitor patient health by taking vital signs, observing patient conditions, and reporting changes promptly to supervising healthcare professionals.
• Administer medications and perform catheter care following medical directives and home healthcare regulations.
• Maintain accurate medical documentation within Electronic Health Record (EHR) systems while ensuring compliance with HIPAA and patient confidentiality standards.
• Support clients with memory care needs, dementia care, Alzheimer's care, and hospice or end-of-life care with patience and compassion.
• Assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a safe and comfortable environment.
• Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and ensure client satisfaction.

Qualifications

• Valid CNA certification required; prior experience working in assisted living facilities, nursing homes, or home healthcare preferred.
• Demonstrated experience working with individuals with disabilities or developmental disabilities is highly valued.
• Knowledge of medical terminology, medication administration, catheter care, vital signs measurement, and basic life support (BLS) or CPR certification.
• Familiarity with EMR/EHR systems for documentation; experience with home health agency operations is a plus.
• Ability to perform heavy lifting safely and use patient transfer equipment such as Hoyer lifts.
• Strong communication skills with a caring attitude; commitment to maintaining patient confidentiality under HIPAA guidelines.
• Flexibility to work PRN AM hours between 8am to 2pm ; valid driver’s license and reliable transportation preferred.

Pay: $20.00 - $24.00 per hour
