PACE Place provides Medical, Rehabilitation, In-Home Assistance, Transportation, Social Work, and Day Center services for patients in our care.

Position Summary

The Certified Nursing Assistant (CNA) provides personal care and support to participants in an adult day center setting, ensuring comfort, hygiene, safety, and active engagement in scheduled activities. This position supports daily operations under the supervision of the Activities Coordinator and functions as part of an interdisciplinary care team. Responsibilities include facilitating activities, delivering direct care in accordance with participant care plans, and maintaining a safe, clean environment.

Shift: Mon - Fri 8 am-5 pm

40 hours a week/8 hour shifts

Located in the PACE Day Center on Ramona Blvd, Jacksonville 32205

Essential Functions

• Provide personal care and assistance with activities of daily living to participants.
• Support and guide participants in organized day center activities.

Deliver light housekeeping services and maintain cleanliness of restrooms and * d spaces. * Assist with meal and snack service in the day center. * Observe and report participant status changes and safety concerns to appropriate staff. * Take and record vital signs as needed. * Assist with bathing and laundry as requested. * Maintain accurate and timely records in the electronic medical record system. * Communicate effectively with team members regarding participant care. * Participate in staff meetings, required trainings, and quality improvement initiatives. * Support and enforce safety protocols, infection control procedures, and HIPAA regulations. * Act within the legal and ethical scope of practice for CNAs * Perform othe...
