Responsibilities

*$2000.00 Sign on Bonus available* The Day Center CNA (Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care.* Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting. * Document care provided in accordance with established policies, guidelines, and applicable regulations. * Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. * Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. * Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. * Collaborate with multidisciplinary teams in care planning for participants. * Monitor appetite and look for changes in functional ability and skin condition. * Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. * Report any variance, accident, or unusual occurrence to the appropriate personnel. * Assist with loading and unloading of participants from vehicles. * Assist the transportation department with participants’ take-home supplies. * Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities. * Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed. * Attend in-service and continuing education programs as scheduled and necessary. * Other du...
