Responsibilities

  • Provide direct supervision and support to residents in accordance with their Individual Service Plans (ISPs).
  • Ensure the health, safety, dignity, and well-being of all residents.
  • Assist residents with activities of daily living (ADLs), personal hygiene, meal preparation, medication reminders/administration (QMAP), transportation, recreation, and community outings as assigned.
  • Implement positive behavior support strategies and approved behavior management plans.
  • Use trauma-informed and de-escalation techniques during behavioral incidents.
  • Complete required documentation, including daily progress notes, medication administration records (MARs), incident reports, and communication logs.
  • Maintain appropriate supervision of residents at all times in accordance with CDHS regulations and agency policies.
  • Participate in team meetings, training, and ongoing professional development.
