Caregiver responsibilities

* Daily personal care such as bathing, grocery shopping, oral care, cooking meals, etc. * Assist and provide help with the daily living activities per the Plan of Care * Track your patient’s condition and/or report to the agency if there is something unusual * Assist patient with daily meals * Basic housekeeping functions * Accompany patient to doctor’s appointments * Assist with essential errands * Communicate with Summit regarding any issue that your patient might have * Follow agency’s policies

Caregiver requirements

* New York Home Health Aide Certificate or Personal Care Assistant Certificate * Government issued ID * Social Security Card * Work permit, if needed * 2 Reference letters * Recent physical * Quantiferon * Rubella and Rubeola lab work * Chain of Custody Drug Screen, no older than 6 months

We would rather have your Flu and/or Covid vaccine records but it’s no longer a requirement.
