Job Summary

As a Home Health Aide & Caregiver, you’ll provide respectful, practical support to clients in their homes and communities. You’ll assist with daily routines, observe changes in well-being, and communicate clearly with clients and care teams. Your impact: Attentive, consistent care helps clients maintain comfort, dignity, and independence while giving families greater confidence in their day-to-day support.

Duties

• Assist with activities of daily living (ADLs), personal care, mobility, and safe patient transfers; use a Hoyer lift when appropriate and follow safe lifting practices.
• Support meal preparation, cooking, cleaning, and laundry according to client needs.
• Observe and report changes, take vital signs when assigned, and complete medical documentation and records using electronic medical record (EMR) or electronic health record (EHR) systems.
• Provide medication assistance or administration only as authorized and within applicable requirements; follow home health regulations, privacy rules, and patient confidentiality practices.
• Offer compassionate support in senior care, dementia and Alzheimer’s care, memory care, hospice, and end-of-life settings as applicable.

Experience

• Patient care, caregiving, home health care assistance, home care, or home health agency experience is valued; assisted living, nursing home, long-term care, live-in, and home and community care experience are also relevant.
• Experience with geriatric patients, individuals with disabilities or developmental disabilities, and patient personal care assistance is helpful.
• Knowledge of medical terminology, catheter care, patient observation, and medical records is preferred.
• Current first aid and cardiopulmonary resuscitation (CPR) skills, including basic life support (BLS), are preferred.
• Bring clear client communication and a respectful approach to protected health information under the Health Insurance Portability and Accountability Act (HIPAA).

Pay: $23.11 - $27.83 per hour

Expected hours: 25.0 per week
