Home Health Aide

Job Summary

We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across diverse settings. In this vital role, you will assist individuals with their daily living activities, support their health and well-being, and promote independence within the comfort of their homes. Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor and observe patients’ health status, recording vital signs such as blood pressure, pulse, respiration, and temperature accurately
• Administer medications and perform catheter care following prescribed medical protocols and home healthcare regulations
• Support clients with meal preparation, light housekeeping, laundry, and maintaining a clean, safe living environment
• Document all patient interactions and medical information using Electronic Health Record (EHR) systems while maintaining strict compliance with HIPAA regulations to ensure patient confidentiality
• Utilize proper patient transfer methods and equipment such as Hoyer lifts or transfer belts to ensure safe mobility assistance
• Provide emotional support and companionship to clients, especially those with dementia or experiencing end-of-life care needs

Skills

• Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring memory care or hospice services
• Strong knowledge of medical terminology, home health care assistance techniques, and home health agency protocols
• Ability to perform patient observation accurately and respond promptly to changing health conditions
• Familiarity with EMR systems for documentation and medical records management
• Excellent communication skills for client interaction and team collaboration
• Knowledge of CPR, First Aid, Basic Life Support (BLS), and other emergency response procedures
• Experience working with clients in assisted living facilities or nursing homes is highly desirable
• Physical ability to lift heavy objects safely and perform tasks requiring prolonged standing or walking
