Home Health Aide - CNA

About this Job: MedStar Health is looking for a PRN - As Needed Day Shift Home Health Aide Per Visit to join our MedStar Health Home Care team! This position requires Virginia CNA License, Active Driver's License and Proof of Car Insurance, along with use of your own reliable transportation.

The ideal candidate will be a Certified Home Health Aide with excellent communication skills.

As a Home Health Aide Per Visit, you will provide personal care for the Agency's patients.

Primary Duties

• Attends Agency regional office and team meetings as required. Attends in-service educational programs. Demonstrates flexibility in adjusting to changes in changes with procedures and in daily schedule. Cleans direct patient care area, as necessary. Identifies safety hazards in the home and reports it to the team's Operations Director or primary case-manager. Reinforces with patient/family ways to remove identified safety hazards in the home.
• Document services in accordance with Agency medical records and timekeeping policies, including the absence of patients from their homes. Documents patient progress reports clearly and concisely for multidisciplinary case conference. Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.
