### The Role

The Home Health Aide (HHA) provides personal care and supportive services to patients in their homes under the supervision of a Registered Nurse (RN) or licensed therapist, in accordance with the patient's individualized Plan of Care. The HHA is responsible for promoting patient comfort, safety, independence, and dignity while adhering to all Agency for Health Care Administration (AHCA), state, and federal regulations.

Essential Duties & Responsibilities

* Provide personal care, including bathing, grooming, dressing, oral hygiene, toileting, and skin care. * Assist patients with ambulation, transfers, positioning, and range-of-motion exercises as directed. * Assist with meal planning, preparation, feeding, and monitoring nutritional intake. * Perform light housekeeping related to the patient's care, including laundry, changing bed linens, and maintaining a clean, safe environment. * Remind patients to take medications as directed (without administering medications unless permitted by law and agency policy). * Observe and promptly report changes in the patient's physical, emotional, or mental condition to the supervising clinician. * Accurately document all care provided, patient observations, and completed visits in accordance with agency policies and regulatory requirements. * Follow the patient's Plan of Care and accept assignments only within the scope of practice. * Maintain patient confidentiality and comply with all HIPAA requirements. * Use proper infection preventi...
