Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to track health status accurately
• Support medication administration following prescribed guidelines while maintaining strict adherence to HIPAA regulations for patient privacy
• Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and safety measures
• Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices to ensure safe patient handling
• Document all patient interactions and medical information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
• Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients

Requirements

• Proven experience in home health care, assisted living facilities, nursing homes or long-term care environments
• Knowledge of medical terminology and procedures related to patient care and documentation
• Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus
