Home Health Aide

key responsibilities:

* As part of the patient care team, works under the guidance and supervision of the staff nurse in providing personal care to the patients * Follows the patient's plan of care for completion of tasks assigned to a home health aide by the registered nurse or other appropriate skilled professional * Maintains an open communication process with the patient, representative (if any), caregivers, and family * Demonstrates competency with assigned tasks * Complies with infection prevention and control policies and procedures * Reports changes in the patient's condition Honors patient rights *

Required Certifications and Licensures

• CNA (Certified Nursing Assistant) or HHA certification from accredited 75-hour program
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
• BLS (Basic Life Support)

Education

* Preferred: High School Diploma or equivalent

Experience

* Preferred 1 year Home Care
