Home Health and Hospice Aide - PRN / Full Time

Job Overview

The aide is a crucial pillar of home care and as such requires a strong level of understanding of pt declining condition and how best to assist them. This position requires a compassionate individual who can offer guidance, comfort, and understanding while offering hygiene and home making assistance to help the pt. The aide will work closely with patients, families, and healthcare teams to ensure holistic care is offered.

Duties

• Provides direct patient care to patient under direction of the RN and according to the Aide Plan of Care.
• Provides necessary skills to assist the patient with safe transfers and ambulation per the company policy
• Provides necessary skill to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care
• Practices accepted infection control principles. Provide a clean, safe, and comfortable environment
• Promotes positive, supportive, respectful communication to patient/family and other employees
• Provides an environment which promotes respect for patient, privacy, and property
• Provides skills necessary to assist the patient with proper nutrition and adequate fluid intake
• Contributes to the management and efficient operation of the Company and demonstrate effective time management skills.
• Provides skills necessary to perform treatments and procedures according to the Company policy
• Promotes the Hospice philosophy and administrative policies to ensure quality of care

Skills

• Strong communication skills to effectively interact with patients, families, and healthcare staff.
• Proficient in writing clear documentation of patient interactions and care plans.
• Experience in hospice care settings is preferred for understanding end-of-life issues.
• Capability in crisis management to handle emergencies with sensitivity and professionalism.

Job Types: Part-time, PRN

Pay: $30.00 - $40.00 per hour
