Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; document findings accurately using Electronic Health Record (EHR) systems
• Administer medications and perform catheter care following medical orders and safety protocols
• Support patients with memory care needs, including Alzheimer's and dementia care, ensuring a safe and comforting environment
• Utilize patient transfer methods such as Hoyer lifts or other assistive devices to ensure safe mobility and prevent injury
• Maintain strict adherence to HIPAA guidelines by safeguarding patient confidentiality and medical records
• Provide emotional support and effective communication to patients and their families, fostering trust and reassurance during home healthcare visits
• Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment for clients
• Follow home health care assistance protocols while complying with all relevant regulations including hospice care, end-of-life support, and geriatric patient needs

Experience

• Prior experience working with individuals with disabilities or in assisted living environments is highly preferred
• Demonstrated experience in geriatrics or long-term care settings such as nursing homes or home healthcare agencies
• Knowledge of medical terminology, patient personal care assistance, and medical documentation practices
• Hands-on experience with home health equipment like Hoyer lifts or other patient transfer devices
• Proven ability to work effectively with geriatric patients, including those with dementia or Alzheimer's disease
• Familiarity with EMR/EHR systems for accurate record keeping and documentation
• Certification in First Aid and Basic Life Support (BLS), including CPR training is required or highly desirable
• Experience working within home & community care settings that involve heavy lifting or physical caregiving tasks
