Hospice Certified Home Health Aide - Hospice

The Hospice Certified Home Health Aide (CHHA) provides compassionate, person-centered care to hospice patients in their homes or preferred care settings. Responsibilities include assistance with daily living, personal care, safety, and vital signs. The CHHA follows an individualized plan of care, communicates closely with the hospice care team, and maintains HIPAA, infection control, and regulatory standards while promoting patient comfort, dignity, and quality of life.

KNOWLEDGE/EXPERIENCE: Sufficient knowledge, effective communication skills, and experience to demonstrate ability to work autonomously. Must demonstrate professionalism while working with patients in their homes. Prefer at least one-year of long term care experience or prior home health or /hospice experience

EDUCATION: High school diploma or GED preferred.

LICENSE/CERTIFICATION/REGISTRY: Current Montana License as a Certified Nursing Assistant (CNA), BLS, Valid drivers license.
