Hospice Certified Nursing Assistant (CNA)

Humanitas Hospice - is looking for Certified Nursing Assistants (CNA). $1000 sign-on bonus.

You will be spending about an hour with each patient helping with ADLs and Companionship. You will be seeing 4 to 7 patients per day. Reliable Mode of Transportation is Required (we do reimburse mileage)

If you're new to being a CNA we have a training / shadow program for you.

Humanitas Health Services is serving mainly the communities of Bucks, Montgomery and Philadelphia providing Hospice services in Home and Long Terms / Skilled and Rehabilitation Facility settings. We provide full health benefits.

Additional Languages are a Plus.

DUTIES

The hospice CNA is responsible for providing personal care to hospice patients based on plan of care.

RESPONSIBILITIES

• Perform personal care (bathing, dressing, toileting, feeding, skin care, etc) according to the Plan of Care.
• Document care provided according to agency policy.
• Report any deviations from the Plan of Care to the Case.
• Provide proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence. Reports any changes/areas of concern to Case Manager.
• Assist with ambulation, transfers, range of motion if documented on the Hospice Aide Plan of Care.
• Provide companionship.
• Use Durable Medical Equipment safely.
• Participate in IDG meetings, as appropriate.

• TB Test will need to be done before the start of patient care
