3 twelve-hour shifts per week | Every other weekend rotation (2 weekends every 6 weeks) | One winter holiday and one summer holiday annually

Transitions LifeCare is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our Hospice Home team on the night shift. In this role, you will provide hands-on personal care and support to hospice patients while helping ensure their comfort, dignity, and quality of life. You will work closely with nurses and the interdisciplinary team in a supportive, mission-driven environment focused on exceptional end-of-life care.

What You'll Do

* Provide safe, compassionate personal care to patients in accordance with their individualized plan of care. * Perform the full range of Certified Nursing Assistant duties. * Complete accurate, thorough, and timely documentation of patient care and assigned responsibilities. * Observe and report changes in patient condition and communicate concerns regarding patients or families to nursing staff promptly. * Participate in interdisciplinary team meetings and care planning as appropriate. * Assist in maintaining an organized supply room, monitoring inventory, stocking supplies, and ordering materials as directed. * Support daily Hospice Home operations through tasks such as: + Patient laundry + Food preparation + Cleaning patient rooms following discharge + Cleaning durable medical equipment (DME) + Light housekeeping in family and common areas * Collaborate with the interdisciplinary group (IDG) and other Transitions LifeCare depar...
