Full-time/Part-time 0.7 FTE (hrs/week) | Day Shift | Monday-Friday | 8:00am-4:30pm

Includes Weekend (every 12th) and Holiday (3/year) rotations

How You’ll Make a Difference (Essential Functions)

The CNA Homecare (HC) is responsible for patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, and documenting care. The CNA in home care (IHC) provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations and policies.

Key Responsibilities

* Assist Patients with Activities of Daily Living + Assist with ambulation, changing position, and moving to/from sitting and standing. + Help with bathroom functions like toileting and incontinence care. + Help bathe, shower, dress, provide oral care, and groom. + Assist with eating and drinking. + Perform post mortem care. + Obtain vital signs. Assist with treatments. + Anticipate and respond to patient needs. * Provide homemaker assistance + Perform light housekeeping such as changing linens, tidying room, doing laundry, and removing trash. + Order and deliver patient supplies. + Help prepare simple meals. * Communication + Read, understand, and follow the plan of care.
