In-Home Care Specialist - CNA/ In-Home Aide

Health Connect HomeCare Solutions is looking for dependable, warmhearted In-Home Care Specialists to join our growing team. We’re a nurse-owned non-medical home care agency built on the belief that seniors and adults deserve care that feels personal, not clinical, and we’re looking for CNAs and In-home Aides who share that same heart for people.

In this role, you’ll support clients in their own homes with day-to-day needs like bathing, dressing, meal preparation, light housekeeping, and companionship. You’ll be someone our clients look forward to seeing, someone their families trust completely. We’re building a team of In-Home Care Specialists who take pride in showing up on time, treating every client with dignity, and going the extra mile without being asked.

Duties

• Assist clients with personal care including bathing, grooming, dressing, and toileting, always with dignity and respect
• Support safe mobility and transfers, including assistance in and out of bed, chairs, and the shower
• Prepare meals in accordance with client preferences and dietary requirements
• Provide medication reminders as outlined in the client’s care plan (administration of medication is not included in this role)
• Perform light housekeeping duties to maintain a clean, safe home environment, including laundry, dishwashing, and general tidying
• Provide meaningful companionship and engagement, including conversation and participation in daily activities
• Accompany clients to medical appointments and assist with errands as needed
• Monitor and report changes in the client’s condition or wellbeing to the care team promptly
• Follow individualized care plans and communicate consistently with agency staff regarding client status
• Maintain accurate and timely documentation of each visit
