About the Role

We are seeking a compassionate and skilled Home Health Caregiver to join our team of caregivers. As a Home Health Caregiver, you will play a vital role in providing one-on-one care to our clients in the comfort of their own homes. Your responsibilities will include:

• Providing assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
• Assisting with medication management
• Maintaining a clean and safe living environment
• Encouraging independence and supporting clients in their daily routines
• Communicating effectively with clients, families, and other healthcare professionals
• Developing and implementing personalized care plans in collaboration with our interdisciplinary team
