About the Role

As an In-Home Caregiver (Caregiver), you will have the opportunity to make a meaningful difference in the lives of Veterans and their families. You will provide personalized care and support to help them maintain their independence and live life to the fullest. Your responsibility will include:

• Assisting with daily living activities such as bathing, dressing, and grooming
• Managing medication schedules and providing medication reminders
• Preparing meals and snacks
• Accompanying clients to appointments and providing transportation
• Engaging in conversations, playing games, and participating in activities to stimulate mental and emotional well-being
• Maintaining accurate records and reporting any changes or concerns to the care team

Responsibilities

• Provide high-quality, patient-centered care to assigned clients
• Develop and implement individualized care plans that meet the unique needs of each client
• Collaborate with interdisciplinary care teams to ensure seamless transitions and continuity of care
• Communicate effectively with clients, families, and healthcare professionals
• Maintain a safe and hygienic environment for clients and staff

Requirements

• High school diploma or equivalent required
• 2+ years of experience in caregiving
