Duties

• Provide attentive personal care assistance, including helping clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support
• Monitor patient health through observation of vital signs and overall condition, promptly reporting any concerns to healthcare professionals
• Assist with medication administration following prescribed schedules and medical documentation protocols
• Support clients with memory care needs, including Alzheimer’s and dementia care strategies to ensure safety and comfort
• Operate electronic health record (EHR) systems and medical records management with accuracy and confidentiality in compliance with HIPAA regulations
• Perform light housekeeping duties such as cleaning, laundry, meal preparation, and maintaining a safe living environment
• Use patient transfer methods and equipment like Hoyer lifts safely to assist with mobility or repositioning as needed
• Administer catheter care and assist with other specialized medical procedures under supervision or as trained
• Maintain patient confidentiality at all times while communicating effectively with clients and their families about care plans and progress
• Follow home health care regulations, including safety protocols like first aid, basic life support (BLS), CPR, and emergency response procedures

Experience

• Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred
• Demonstrated experience providing geriatric care or working with individuals with developmental disabilities or dementia is essential
• Familiarity with medical terminology, medication management, vital signs monitoring, and end-of-life care practices is required
• Proven ability to handle heavy lifting safely and operate patient transfer equipment such as Hoyer lifts or similar devices
• Experience working within home health or hospice care environments ensures understanding of relevant regulations and documentation standards
• Strong communication skills for client interaction and team collaboration are necessary for success in this role
• Live-in employment experience is advantageous but not mandatory; however, a commitment to providing consistent 24-hour support is essential
