Per Diem LPN - Caregiver Trainer (Home Visits)

We are looking for a compassionate and reliable Licensed Practical Nurse (LPN) to travel to patients' homes and train caregivers on proper care techniques. This is a per diem position offering flexible scheduling and competitive per-visit pay.

What You'll Do

• Travel to patients' homes to provide hands-on caregiver training
• Teach caregivers proper care techniques and best practices
• Ensure caregivers are confident and prepared to safely support patients
• Complete required documentation after each visit
• Communicate with our clinical team regarding patient and caregiver needs

Qualifications

• Active LPN license
• Reliable vehicle and valid driver's license
• Comfortable traveling to patients' homes
• Strong communication and teaching skills
• Compassionate, professional, and dependable
