Gray Birch is seeking a compassionate, organized, and resident-focused Resident Care Coordinator (CNA) to support the delivery of exceptional care and services to our residents. This role serves as a key liaison between residents, families, nursing staff, and interdisciplinary team members to ensure resident needs are met, care plans are coordinated, and residents experience the highest quality of life possible.

What You’ll Do

• Greets patients and visitors. Answers, screens, and routes telephone calls. Calls codes, pages and overhead announcements as requested.
• Performs responsibilities of certification or licensure. Takes responsibility for, or provides guidance to team members on, the handling of complex or novel issues.
• Assists with processing patient admissions, transfers and discharges. Assembles and maintains patient records and charts.
• Performs needed clerical tasks, including copying, faxing, filing, and ordering/obtaining supplies.
• Operates a motor vehicle to conduct MaineGeneral Health business.

What You'll Bring

• Active Certified Nursing Assistant (CNA) license in the State of Maine
• Valid State of Maine driver's license
• Strong interpersonal communication and organizational skills
• Ability to work collaboratively with residents, families, and healthcare team members
