RN/LPN Assisted Living & Memory Care Manager (Rockwood South Hill Campus)

Job Overview

Rockwood Retirement Communities is seeking a dedicated and experienced Assisted Living & Memory Care Manager to oversee operations at our senior care campus specializing in assisted living and memory care services. The ideal candidate will be able to manage the care in AL and MC, and will possess a strong background in long-term care, nursing home administration, and dementia care, with a passion for enhancing the quality of life for residents. This leadership role requires strategic oversight of daily operations, staff supervision, regulatory compliance, and resident-centered care planning. The manager will play a vital role in fostering a compassionate environment that promotes independence, safety, and well-being for all residents.

Shift type

Full-time (Schedule TBD)

• + Shift differential for Evening and/or Night Shift
• +Pay in lieu of Benefits (Full-time, only)

Responsibilities

• Resident functional assessments and service plans completed at least every 6 months.
• Coordination of all admissions and discharges in Assisted Living and communication of all changes in resident status to the clinical transitions team.
• Parnering with HR with recruiting, hiring, orientation, mentoring, evaluating and maintaining education structure for new and current team members.
• Care Conference coordination.
• Long Term Care insurance evaluations.
• Monitor the successful engagement of activities for residents with cognitive impairment within your area of supervision.
• Lead at resident-directed approach to assessment and service/care planning.
• Work with the Interdisciplinary Team to communicate with residents and families before plan of care is changed (whenever possible) or when there is a change in the resident’s condition.
