About Us

Gifted Iris Healthcare LLC is an Oregon Health Authority-licensed In-Home Care Agency providing Personal Care, Housekeeping, and Direct Nursing Services (DNS) to individuals through Oregon Medicaid (APD and ODDS programs) and private pay. We are growing our caregiver team to meet increasing demand for compassionate, reliable in-home support across the Portland metro area.

Job Summary

We are seeking dependable, compassionate Caregivers to provide non-medical personal care and light housekeeping support to clients in their own homes. You will help clients maintain independence, dignity, and quality of life through activities of daily living (ADLs) and instrumental activities of daily living (IADLs).

Responsibilities

• Assist clients with personal care, including bathing, grooming, dressing, toileting, and mobility
• Provide light housekeeping: laundry, meal preparation, dishes, tidying living spaces
• Assist with medication reminders (as permitted)
• Perform delegated nursing tasks (such as medication administration) for select clients, when trained and authorized under Oregon RN delegation requirements
• Provide companionship and emotional support
• Accompany clients to appointments or errands as needed
• Observe and report any changes in client condition to the office/care coordinator
• Accurately document services provided using Electronic Visit Verification (EVV) via mobile app or tablet
• Follow individual client care plans and maintain client confidentiality
• Communicate professionally with clients, families, and Gifted Iris office staff
