Responsibilities

• Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Provide compassionate dementia, Alzheimer’s, memory care, and behavioral management assistance tailored to individual needs
• Maintain a clean and safe environment through light housekeeping tasks such as cleaning resident rooms and living areas.
• Document care activities accurately using WI EVV and report any changes or concerns promptly punching in and out VIA EVV system aquiring members EVV verification.
• Support members social engagement by encouraging and engaging that promote mental stimulation and emotional well-being and companionship.
• Drive residents when necessary for appointments or community outings while adhering to safety protocols. *Full insurance and Valid DL*.
• Pass Caregivers back ground.

Skill

• Strong understanding of dementia, Alzheimer’s, memory care, and behavioral management techniques PLUS
• Experience working with individuals with developmental disabilities or disabilities in personal home and community settings.
• Familiarity with home & community care practices accurate documentation
• Excellent communication skills to collaborate effectively with team members, Members and Team Lead
