Overview

We are seeking a dedicated and compassionate Home Health Aide & Caregiver to join our team. In this role, you will deliver essential in-home care services to clients with diverse needs, including those requiring memory care, hospice support, and assistance with activities of daily living (ADLs). The ideal candidate will possess a strong commitment to patient well-being, excellent observational skills, and the ability to work effectively within home healthcare regulations. This position offers an opportunity to make a meaningful difference in the lives of our clients through attentive and respectful caregiving.

Responsibilities

• Provide personal care and assistance with activities of daily living such as bathing, dressing, grooming, and mobility support.
• Monitor and document patient conditions regularly, including vital signs and changes in health status.
• Assist with medication administration following prescribed protocols while maintaining compliance with HIPAA and home health regulations.
• Support patients with specialized needs such as Alzheimer's care, dementia care, and working with individuals with developmental disabilities.
• Operate medical equipment including Hoyer lifts, catheters, and EMR (Electronic Medical Records) systems for accurate documentation.
• Prepare nutritious meals and assist with light housekeeping tasks such as cleaning and laundry to promote a safe living environment.
