Job Overview

We are seeking a dedicated and compassionate Home Health Aide & Caregiver to join our team. In this vital role, you will deliver high-quality in-home care services, supporting clients with their daily needs while promoting independence and well-being. The ideal candidate will possess a strong background in patient care, demonstrate excellent communication skills, and be committed to upholding home healthcare standards.

Duties

• Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance
• Monitor and document patient observations, vital signs, and medical conditions accurately using electronic health record (EHR) systems
• Provide specialized care such as Alzheimer's, dementia, memory care, hospice, and end-of-life support
• Administer medications following prescribed protocols while maintaining compliance with HIPAA regulations
• Support clients with medical procedures including catheter care, wound care, and assisting with medical documentation
• Prepare nutritious meals and assist with light housekeeping tasks such as cleaning and laundry to ensure a safe living environment
• Utilize assistive devices like Hoyer lifts safely and effectively to prevent injury during patient transfers

Experience

• Prior experience in assisted living facilities, nursing homes, or home healthcare agencies is highly desirable
• Demonstrated experience working with individuals with developmental disabilities or geriatrics
