Duties

• Assist clients with personal care tasks such as bathing, dressing, grooming, and mobility transfers using patient transfer methods and equipment like Hoyer lifts.
• Monitor and record vital signs, medication administration, and medical documentation accurately in EMR (Electronic Medical Records) or EHR systems.
• Support clients with ADLs including meal preparation, light cleaning, laundry, and other home health care assistance to promote independence.
• Observe patient conditions closely, noting changes in health status or behavior related to dementia, Alzheimer's disease, or other cognitive impairments.
• Follow home healthcare regulations and HIPAA guidelines to maintain patient confidentiality at all times.
• Provide emotional support and companionship, especially for clients in hospice or end-of-life care settings.
• Respond promptly to client needs using appropriate patient transfer methods and ensure safety during all activities.

Experience

• Previous assisted living or long-term care experience working with geriatric patients is highly preferred.
• Demonstrated experience working with individuals with developmental disabilities or dementia care is a plus.
• Knowledge of medical terminology, catheter care, and basic life support (CPR/First Aid) is required.
