Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary.
• Monitor patients' vital signs and observe health status to report changes or concerns promptly.
• Prepare nutritious meals tailored to individual dietary needs and preferences, ensuring proper nutrition and hydration.
• Support patients with memory care needs, including dementia and Alzheimer's disease, through specialized techniques and patience.
• Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment for clients.
• Collaborate with healthcare professionals, family members, and support teams to develop and implement effective care plans.

Requirements

• Proven experience working in home health, assisted living facilities, nursing homes, or group homes is highly desirable.
• Strong patient observation skills with the ability to recognize changes in health or behavior promptly.
• Experience supporting individuals with developmental disabilities, neurodevelopmental disorders such as autism, or geriatrics is preferred.
• Ability to perform manual handling tasks safely, including heavy lifting when necessary for patient transfers or mobility assistance.
