Duties

• Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary
• Monitor and observe patients’ conditions by recording vital signs, noting changes in health status, and reporting findings accurately in electronic health record (EHR) or medical documentation systems
• Provide medication reminders and assist with medication administration following home health care regulations and medical terminology guidelines
• Support patients with specialized needs such as Alzheimer’s care, dementia care, memory care, hospice care, or end-of-life support with sensitivity and professionalism
• Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients
• Maintain patient confidentiality in compliance with HIPAA regulations while communicating effectively with clients and healthcare teams
• Use proper patient transfer techniques and equipment such as Hoyer lifts or other mobility aids to ensure safety during patient transfers

Experience

• Prior experience working with individuals with disabilities, developmental disabilities, or geriatric patients in settings such as assisted living facilities or nursing homes is highly preferred
