Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods such as Hoyer lifts when necessary
• Monitor patient observations by regularly checking vital signs and reporting any changes promptly
• Administer medications following medical documentation protocols and home healthcare regulations
• Support clients with meal preparation, light housekeeping, laundry, and cleaning to maintain a safe and comfortable environment
• Document medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining patient confidentiality in compliance with HIPAA standards
• Provide specialized care for clients with dementia or Alzheimer’s disease, including memory care techniques and behavioral management strategies
• Assist with end-of-life care and hospice support, ensuring comfort and emotional well-being for patients and their families

Qualifications

• Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred
