Responsibilities

• Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
• Monitor and document vital signs, medication administration, and changes in patient condition using Electronic Health Record (EHR) systems and medical documentation protocols.
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments, including memory care and behavior management strategies.
• Support patients with medical procedures such as catheter care, wound care, and end-of-life care while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards.
• Assist with meal preparation, light housekeeping tasks like cleaning and laundry, and ensure a safe environment for clients by following safety protocols including first aid and basic life support (BLS) procedures.
• Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and update medical records accurately.
• Support clients during transfers or mobility exercises using proper patient transfer methods to ensure safety for both patient and caregiver.
