Home Health Aide

Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, grooming, dressing, and mobility support
• Monitor and observe patient conditions, documenting vital signs and changes in health status accurately using Electronic Health Record (EHR) systems
• Administer medications and perform catheter care following medical instructions and home healthcare regulations
• Support patients with meal preparation, light housekeeping, laundry, and maintaining a clean living environment
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or developmental disabilities, including memory care and end-of-life support
• Safely transfer patients using patient transfer methods such as Hoyer lifts or other assistive devices while ensuring patient safety and comfort
• Communicate effectively with patients, families, and healthcare teams to coordinate care plans while maintaining strict confidentiality in accordance with HIPAA guidelines

Requirements

• Proven experience working as a Home Health Aide or in assisted living, nursing homes, or hospice care environments
• Strong knowledge of medical terminology, vital signs monitoring, medication administration, and basic life support (BLS) including CPR certification
