Duties

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
• Monitor and document patient health status by observing vital signs, recognizing changes in condition, and maintaining accurate medical records using Electronic Health Record (EHR) systems.
• Administer medications following prescribed instructions while adhering to home healthcare regulations and ensuring patient safety.
• Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory care needs, including engaging activities and environment safety measures.
• Support patients during end-of-life or hospice care with compassion, dignity, and respect for their wishes.
• Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment.
• Communicate effectively with clients and their families to update on health status and care plans while maintaining strict confidentiality in accordance with HIPAA standards.
