Responsibilities

• Assist residents and patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor and observe patient conditions, recording vital signs such as blood pressure, pulse, temperature, and respiration accurately in electronic health record (EHR) systems
• Support medication administration under supervision, ensuring proper documentation following home healthcare regulations and HIPAA guidelines
• Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies
• Assist with medical procedures such as catheter care, wound dressing changes, and hospice/end-of-life care needs
• Support safe transfer techniques using equipment like Hoyer lifts or other assistive devices while adhering to safety protocols for heavy lifting and patient handling
• Maintain a clean environment by assisting with laundry, cleaning duties, meal preparation, and ensuring infection control standards are met

Requirements

• Valid certification as a Nursing Assistant or Caregiver with active state licensure or registration
• Proven experience working with geriatric patients in assisted living facilities, nursing homes, or home healthcare settings
