Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
• Support patients with specialized care needs such as Alzheimer’s disease, dementia, or memory care requirements.
• Use patient transfer methods safely, including Hoyer lifts and other assistive devices; ensure proper patient handling techniques.
• Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry and meal preparation while adhering to infection control standards.
• Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met.
• Follow all home health care assistance guidelines, including hospice or palliative care procedures when applicable.

Qualifications

• Valid Certified Nursing Assistant (CNA) certification or equivalent caregiving credentials.
• Proven experience working with geriatric patients or in assisted living settings; experience in nursing homes or long-term care is highly valued.
• Familiarity with home health agency operations, home & community care regulations, and documentation practices using EMR/EHR systems.
• Strong patient observation skills with the ability to recognize changes in condition or behavior promptly.
