Responsibilities

• Deliver comprehensive patient care including assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
• Assist with specialized care needs such as dementia and Alzheimer’s care, hospice support, end-of-life assistance, and managing patients with developmental disabilities or geriatrics.
• Maintain patient safety through diligent observation of patient conditions, responding promptly to changes in health status, and adhering to home healthcare regulations and HIPAA confidentiality standards.
• Support clients’ nutritional needs by preparing meals aligned with dietary restrictions and assisting with feeding when necessary.
• Facilitate effective communication with clients and their families to ensure personalized care plans are followed accurately while maintaining patient confidentiality.
• Perform light housekeeping duties including cleaning, laundry, and meal preparation to promote a safe and comfortable living environment.
