Duties

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote comfort and independence.
• Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) systems.
• Support medication administration following prescribed protocols while ensuring compliance with HIPAA regulations for patient privacy.
• Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care techniques and behavioral management strategies.
• Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively.
• Assist with mobility using tools like Hoyer lifts or transfer belts to prevent falls and injuries.
• Perform light housekeeping duties, including cleaning, laundry, meal preparation, and maintaining a safe environment to enhance overall well-being.
• Collaborate with healthcare teams to document medical records accurately and ensure continuity of care across home & community settings.
• Support hospice patients by providing compassionate end-of-life care aligned with their wishes and needs.
