You will be a vital part of enhancing patient well-being by delivering high-quality assistance with daily activities and health monitoring, all while working in a supportive and energetic environment committed to excellence.

Responsibilities

• Provide compassionate assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health by observing vital signs, managing catheter care, and assisting with medication administration in accordance with medical plans
• Support patients with mobility and transfers using patient transfer methods and equipment such as Hoyer lifts or other assistive devices
• Maintain detailed medical documentation within Electronic Health Record (EHR) systems, ensuring accuracy and confidentiality in compliance with HIPAA regulations
• Assist with meal preparation, light housekeeping, laundry, and cleaning to promote a safe and comfortable living environment
• Observe and report changes in patient condition promptly to healthcare professionals, ensuring timely interventions
• Engage with patients experiencing dementia or Alzheimer’s disease through specialized memory care techniques and compassionate communication

Skills
