Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient condition through observation of vital signs and overall well-being
• Administer medications and document medical records accurately using Electronic Health Record (EHR) systems
• Support clients with specialized care needs such as Alzheimer’s, dementia, hospice, and end-of-life care
• Operate medical equipment like Hoyer lifts and perform catheter care with precision and compassion
• Prepare nutritious meals, assist with cooking, and manage light housekeeping tasks including cleaning and laundry
• Follow home healthcare regulations and maintain strict adherence to HIPAA privacy standards

Skills

• Strong patient observation skills to detect changes in health status promptly
• Experience working with individuals with developmental disabilities or geriatrics preferred
• Knowledge of medical terminology and medication administration procedures
• Proficiency in EMR/EHR systems for accurate medical documentation
• Ability to assist with complex tasks such as hospice care, memory care, and heavy lifting using appropriate equipment
• Excellent communication skills for working effectively with clients, families, and healthcare teams
• Certification in CPR, First Aid, Basic Life Support (BLS), and Home & Community Care preferred
