Job Summary

As a Caregiver/Personal Care Assistant serving Shirley, Holbrook, and Center Moriches in Suffolk County, you’ll provide attentive home care tailored to each client’s needs. You’ll support daily routines, communicate changes, and help clients feel safe and respected in their homes.

Responsibilities

• Assist with activities of daily living (ADLs), personal care, meal preparation, cooking, light cleaning, and personal transportation.
• Follow care plans; support medication administration as authorized, and observe and monitor clients, including noting vital signs when required.
• Provide safe transfers and manual handling, using equipment such as a Hoyer lift when appropriate and authorized.
• Offer direct support and patient care for older adults and people with disabilities, including clients with dementia, Alzheimer’s disease, autism, intellectual disabilities, or other developmental and neurodevelopmental conditions.
• Protect privacy, follow the Health Insurance Portability and Accountability Act (HIPAA) and residents’ rights, and respond appropriately to safety concerns. Your impact: Consistent, respectful care helps clients maintain daily routines and supports families’ peace of mind.

Experience

• Caregiving, home care, home health, or home and community care experience is relevant; experience in senior care, geriatrics, hospice, assisted living, group homes, nursing homes, or long-term care is welcome.
• Experience with memory care, dementia care, patient transfers, patient observation, and client communication is valuable.
• First aid and cardiopulmonary resuscitation (CPR) skills are preferred; familiarity with nursing practices and patient monitoring is helpful.
• Ability to perform required lifting and follow safe handling practices is important.

Requirements

• Must be 18 years old and above
• At least one year of experience working with seniors in a home setting (required)
• Must have a Driver's License (required)
• Must have a car (preferred)
• Personal care and incontinence care experience (required)
• English proficiency and excellent communication skills
• Two professional references required
• Must be able to pass background check
• HHA, CNA and PCA (preferred)

Pay: From $19.65 per hour
