Caregiver/Personal Care Assistant

Schedule: Monday & Tuesday, 9:00 AM–1:30 PM; Thursday, 9:00 AM–9:00 PM. Occasional weekend hours may be available with advance notice and can be picked up based on caregiver availability.

Job Overview

New Nova Home Care is seeking a compassionate and dependable Caregiver/Personal Care Assistant to provide one-on-one care for a client with Alzheimer’s who requires total-care assistance. The ideal candidate is patient, attentive, and committed to promoting the client’s safety, comfort, dignity, and quality of life.

Alzheimer’s and total-care experience is preferred, but we are willing to train the right candidate. Reliable transportation is required.

Responsibilities

• Provide total-care assistance with ADLs, including bathing, dressing, grooming, toileting, incontinence care, repositioning, and transfers
• Support assisted ambulation approximately every 2 hours per care plan; client can take some steps with caregiver assistance
• Assist with meals, hydration, feeding as needed, and medication reminders within caregiver scope
• Provide companionship, supervision, reassurance, and dementia-appropriate redirection
• Perform client-related light housekeeping, laundry, and linen changes
• Observe changes in health, behavior, mobility, and overall condition and promptly report concerns
• Provide daily updates to the New Nova nurse/on-call nurse and escalate concerns or changes as needed
• Accurately complete required care documentation and follow the individualized care plan
• Maintain the client’s safety, dignity, privacy, and confidentiality at all times
