Transitions At Home

is seeking a compassionate, dependable PRN Caregiver to provide exceptional in-home care for clients in the Lyndon Station area.

If you have a passion for helping others maintain their independence and quality of life, we'd love to hear from you. As a caregiver, you'll make a meaningful difference by helping clients remain safe, comfortable, and independent in their own homes.

Primary Responsibilities

• Assist with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene
• Prepare meals and assist with cooking
• Perform light housekeeping and laundry
• Provide safe transfers and mobility assistance
• Offer companionship and meaningful social engagement
• Brighten each client's day with friendly, compassionate support

Preferred Qualifications

• Experience with catheter and ostomy care is a plus
• Willingness to learn—we are happy to train the right candidate
• Compassionate, reliable, and patient demeanor
• Ability to work independently while providing high-quality care
• Strong communication and interpersonal skills

Hours: 10 Hours a Week

* Monday 10am-1pm * Friday 10am-4pm * No Holidays * No Weekends
