Certified Home Health Aide/ CNA

Job Summary

Duties

• Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support, including the safe use of Hoyer lifts and other assistive devices.
• Monitor vital signs and observe changes in patient conditions, accurately documenting findings within electronic health record (EHR) systems such as Epic, Cerner, or eClinicalWorks.
• Maintain accurate medical documentation of patient interactions and interventions using EMR/EHR systems while ensuring compliance with healthcare regulations.

Requirements

• Valid CNA certification with current licensure in the relevant state or region.
• Strong understanding of HIPAA regulations to ensure patient confidentiality at all times.
• Demonstrated ability to work effectively with patients of all ages—including children—and individuals with disabilities or behavioral health needs.
• Knowledge of specialized areas such as Alzheimer's/dementia/memory care or hospice services is advantageous.
• Excellent communication skills coupled with compassionate caregiving abilities to foster trust and comfort among patients and their families.
